We’re happy to assist you! We’ve made our process extremely easy. If you already have an existing primary care doctor than all you need to do is request to be referred to us.

If you don’t have a primary care doctor no need to worry. Our new self-referral process is easy to follow and we’ve provided instructions below.

Self Referral

• Patients interested in being assessed by an Atma Physician by self-referral may either download a copy of our self-referral form and either fax, email or drop off in person once completed. Alternatively, patients may complete the self-referral form entirely online
• Upon receipt of the completed referral form our office will then contact you directly to schedule an appointment with a physician

Referral by Doctor

• Patients interested in being assessed by an Atma Physician and are not self-referring should request their doctor to complete our referral form either online or in printed format which may be faxed back
• Upon receipt of the completed referral form our office will then contact the patient directly to schedule an appointment
